Abstract

Background
The coexistence of a molar pregnancy with a viable fetus is rare and poses significant diagnostic and management challenges. Common symptoms include vaginal bleeding, anemia, hyperemesis gravidarum, hypertension, thyrotoxicosis, and disproportionate uterine enlargement to gestational age. While most such cases involve a complete mole, partial molar pregnancies with a coexisting fetus are extremely rare and usually result in miscarriage due to fetal triploidy.

Case Presentation

We report a case of a unbooked 26 year-old woman presenting with severe vaginal bleeding and lower abdominal pain at 16 weeks gestation. Ultrasound confirmed a live fetus and revealed a large heterogeneous cystic mass suggestive of molar changes. She progressed rapidly to expel the fetus that died within few minutes and the placenta while suction evacuation was done to remove the remnant molar pregnancy. Histopathology confirmed a partial hydatidiform mole.

Discussion
The case illustrates the rare occurrence of a partial molar pregnancy coexisting with a viable fetus, complicated by inevitable miscarriage and hemorrhage. Early recognition and multidisciplinary management are critical for optimizing outcomes.

Conclusion
Postpartum recovery was uneventful. ?-hCG levels normalized within a month without intervention, and no evidence of persistent trophoblastic disease was found at six-month follow-up.
